ARTSCAMP LIABILITY & MEDIA RELEASE INFORMATION

Participants in ArtsCamp will be recorded and photographed. I/We, the undersigned, hereby grant The Raymond F. Kravis Center for the Performing Arts, Inc. permission to record and photograph us during the program on June 2-20, 2025. Video and photographs taken during the Intensive will be the sole property of The Raymond F. Kravis Center for the Performing Arts, Inc.

PLEASE READ
(I) Permission is hereby granted for the free use of participant’s name and picture as listed on this form to be used in broadcasts, newspapers, brochures, websites, social media, and any other form of communication to which use may be applied, including video recording and pictures taken during the event, ArtsCamp.

(II) Activities of ArtsCamp from June 2-20, 2025, will be held in the Cohen Pavilion. Participation in the program involves the risk of accidental injury, despite all safety precautions. Having been informed of the activities to be conducted in the program, I, as a participant named herein (or as a parent/legal guardian of the participant), assume all risks and hazards incidental to the activities, and release from responsibility and agree to indemnify and hold harmless The Raymond F. Kravis Center for the Performing Arts, Inc., and their respective officers, instructors, volunteers, and all employees, for any illness or injury to me or my child or family members occurring during his/her/their participation in any activities or use of any facilities at, or conducted by, The Raymond F. Kravis Center for the Performing Arts, Inc.

IN THE EVENT OF AN EMERGENCY
Should an emergency arise, ArtsCamp staff will attempt to contact the child’s parent(s)/guardian(s). If ArtsCamp staff are unable to contact you, the parent(s)/guardian(s), we will attempt to contact your listed emergency contact.
